Q: To whom does Revive Psychological Services provide services?

A: We primarily specialize in treating adolescents and adults in an outpatient setting.

Q: Do you take insurance?

A: Yes we accept major PPO insurances. We do not accept HMO insurance.  Kindly go to request appointment, and tell us what insurance you have and we will call or e-mail you. Mental health coverage varies from plan to plan, so we strongly encourage prospective clients to speak with their insurance provider prior to their first appointment. Alternatively, you can call us and give us the following information and we can check your coverage:

Q. What is your rate for out of pocket payments without insurance?

A: Our rates for one on one sessions vary from depending on the clinician you will be seeing. Rate is determined based on their experience and qualification.

For group therapy our rates are $30 per person per session.

Q: Which forms of payment do you accept?

A: We accept major credit cards, checks and cash.

Q: What are your hours?

A: Revive Psychological Services has office hours Monday through Saturday, including evening hours on the weekdays.

Q: What should I do to prepare for the first session?

A: To prepare for your visit, you can download the forms from the website and fill them out prior to your first visit. Please also bring your doctors/psychiatrist contact information so that we can coordinate care with them.
