Q: To whom does Revive Psychological Services provide services?

A: We primarily specialize in treating adolescents and adults in an outpatient setting.

 

Q: Do you take insurance?

A: Yes we accept all major PPO insurances, BCBS PPO, Aetna PPO, United Healthcare PPO. We do not accept HMO, Meridian or BCBS Community insurance. Mental health coverage varies from plan to plan, so we strongly encourage prospective clients to speak with their insurance provider prior to their first appointment. Alternatively, you can call us and we can check your coverage.

 

 

Q: Steps to scheduling an appointment for new clients?

Step 1: Request an available appointment. 

Step 2: Upon receiving your request, we will send you an e-mail to create a client portal account through a link.  

Step 3: Once you set up your client portal account, we will send client information and insurance forms to your portal.

Step 4: After you fill out those two forms, we can verify whether we accept your insurance. We will then send you other intake forms to sign, again through your

client portal.

Step 5: Upon receiving all the forms, your clinician will reach out to you to confirm your appointment. (All forms are completed online through the client portal)

Step 6: Start your emotional healing and growth journey either in office or through telehealth!

Q. What is your rate for out of pocket payments without insurance?

A: Our rates for one on one sessions vary from depending on the clinician you will be seeing. Rate is determined based on their experience and qualification.

Q: Which forms of payment do you accept?

A: We accept major credit cards, checks and cash. You can check your next appointment and also make payments on your account through your client portal. 

Q: What are your hours?

A: Revive Psychological Services has office hours Monday through Saturday, including evening hours on the weekdays.

Q: What should I do to prepare for the first session?

A: To prepare for your visit, please complete all documents sent to you through your client portal. Your appointment will be cancelled if you do not complete all the forms. If you are having a telehealth session, kindly login to your client portal and meet with your therapist.
